Desmopressin (cas 16679-58-6) (1-deamino-8-D-arginine vasopressin, DDAVP) is a synthetic analogue of the antidiuretic hormone vasopressin, which increases the plasma levels of factor VIII and von Willebrand factor, without inducing clinically relevant side effects, like its parent compound vasopressin. It is used for the prevention and treatment of bleeding in patients with mild hemophilia A and von Willebrand disease. In these patients, desmopressin can secure normal hemostasis, effectively replacing allogeneic plasma-derived or recombinant factors. Desmopressin is also of hemostatic benefit in some patients with platelet function defects. In addition, Desmopressin has been used prophylactically in patients undergoing surgical operations characterized by large blood losses and transfusion requirements (specially cardiac, joint replacement and spinal surgery). Some of these indications for desmopressin have been strengthened by the experience accumulated, others have not been supported by clinical trials or have been superseded by the advent of more efficacious treatments.
