We aimed to det. the effects of tirofiban on oxidative stress and neutrophil-endothelium interaction markers in patients with acute coronary syndromes (ACS). We measured malondialdehyde (MDA), interleukin-6 (IL-6) and sol. endothelial intercellular and vascular adhesion mols. (sICAM-1 and sVCAM-1) on admission, at 48 and 72 h and on 5th day of hospitalization in 34 patients (age 66.5±1.8 years) with ACS. Patients with recurrent angina, changes on the ECG and/or elevated troponin I received i.v. tirofiban for 48 h and the others received normal saline. Baseline concns. of all markers did not differ significantly and compared with placebo, tirofiban infusion markedly reduced MDA, IL-6, sICAM-1 and sVCAM-1 at 48 h (-31±6% vs. 542-78-9 and 144494-65-5 are cas registry numbers of chemicals which are used as reagents here. 84±49%, p = 0.007, -12±14% vs. 23±10%, p = 0.05, -20±6% vs. 36±25%, p = 0.04 and -10±5% vs. 6±5%, p = 0.02, resp.). Relative to baseline, significant redns. were obsd. for all 4 markers at 72 h and day 5 (p < 0.05). Tirofiban potentiates the decline in oxidative stress and may reverse abnormal endothelial activation in patients with ACS. This benefit seems to remain over the following 5 days. .

Anti-GP IIb/IIIa agents may influence platelet thrombi already formed. Blood samples were anticoagulated either by the specific antithrombin Argatroban (100 mM) or by unfractionated heparin (0.1 U/mL). After platelet thrombi were formed on a collagen surface following 6-min perfusion of whole blood obtained from eight adult donors contg. fluorescinated platelets at a wall shear rate of 1,500 s-1, addnl. blood samples from the same donors either contg. or not contg. anti-GP IIb/IIIa agents (abciximab, eptifibatide, or tirofiban) were perfused on these thrombi. The three-dimensional structures of the platelet thrombi were continuously obsd. by laser confocal microscopy equipped with a piezo-elec. motor control unit and recorded. The platelet thrombi started to dissolve after perfusion of blood contg. the anti-GP IIb/IIIa agents, whereas their growth resumed after subsequent perfusion of control blood. Only a single layer of platelets having heights of 3±1 mm, 3±2 mm, and 3±1 mm, resp., could be seen after 6-min perfusion of blood contg. abciximab, eptifibatide, and tirofiban, whereas the initial height of the platelet thrombi of 8±2 mm increased to 11±4 mm after subsequent perfusion of control blood (n = 8). The vol. of the platelet thrombi, which was 3,352±1,045 mm3 before starting the second perfusion, was reduced to 778±102 mm3, 812±122 mm3, and 856±144 mm3 after 6-min perfusion of blood contg. abciximab, eptifibatide, and tirofiban, resp. We have shown in this study that anti-GP IIb/IIIa agents possess the ability to dissolve platelet thrombi. .
