DOCUMENT TYPE: Journal CA Section: 1 (Pharmacology) Dietary phosphate restriction and the oral administration of calcium and aluminum salts have been the principal means of controlling hyperphosphatemia in individuals with end-stage renal disease over the past decade. Although relatively well-tolerated, a large fraction of patients treated with calcium develop hypercalcemia, particularly when administered concurrently with calcitriol, despite a lowering of the dialyzate calcium concn. The authors evaluated the efficacy of crosslinked poly[allylamine hydrochloride] (RenaGel; Geltex Pharmaceuticals, Waltham, MA), a nonabsorbable calcium- and aluminum-free phosphate binder, in a randomized, placebo-controlled, double-blind trial of 36 maintenance hemodialysis patients followed over an 8-wk period. RenaGel was as effective as calcium carbonate or acetate as a phosphate binder. The redn. in serum phosphorus was significantly greater after 2 wk of treatment with RenaGel (6.6±2.1 mg/dL to 5.4±1.5 mg/dL) compared with placebo (7.0±2.1 mg/dL to 7.2±2.4 mg/dL; P = 0.037). There was no significant change in serum calcium concn. in either treatment group. The total serum cholesterol and low-d. lipoprotein cholesterol fraction were significantly reduced in RenaGel-treated patients compared with placebo-treated patients (P = 0.013 and P = 0.003, resp.) without a concomitant redn. in high-d. lipoprotein cholesterol (P = 0.93). 152751-57-0 and 7723-14-0 are just another two chemicals used in this study. There was no difference among recipients of RenaGel and placebo in terms of adverse events. RenaGel is a safe and effective alternative to oral calcium for the management of hyperphosphatemia in end-stage renal disease. .

Hyperphosphatemia is one of the major complications of hemodialysis patients and plays a key role in the pathogenesis of cardiovascular calcification and secondary hyperparathyroidism. Dietary phosphate restriction and removal of phosphate by dialysis are insufficient to control hyperphosphatemia. Therefore, almost all patients undergoing hemodialysis should take oral phosphate binders. Sevelamer hydrochloride (sevelamer) is a novel phosphate-binding polymer that contains neither aluminum nor calcium, and it is not absorbed from the gastrointestinal tract. In rat models with progressive chronic renal insufficiency, in addn. to lowering effects on serum levels of phosphorus, calcium ′ phosphorus product, and parathyroid hormone, dietary treatment of sevelamer can prevent parathyroid hyperplasia, vascular calcification, high turnover bone lesion, and renal functional deterioration. In clin. studies with hemodialysis patients, sevelamer lowers serum phosphorus and calcium ′ phosphorus product without any incidence of hypercalcemia. Switching calcium-contg. phosphate binders to sevelamer can decrease the percentage of hypoparathyroidism and hyperparathyroidism by neg. calcium balance and increased dosage of vitamin D, resp. Sevelamer also decreases serum low-d. lipoprotein cholesterol levels by its bile acid-binding capacity. A long-term clin. study has demonstrated that the progression of coronary and aortic calcification in hemodialysis patients is attenuated by sevelamer. Thus, sevelamer offers the promise of impacting cardiac calcification and thereby reducing morbidity and mortality of hemodialysis patients. .
