New antifungal drugs and new clinical trials: interpreting results may be difficult. In recent years, considerable progress in the management of invasive mycoses in immunocompromised patients has been achieved. However, the prognosis of these infections, in particular those caused by filamentous fungi, continues to be dramatically poor, with mortality rates reaching more than 80% in selected categories of patients. New antifungal drugs in the classes of triazoles and echinocandins, with interesting antimicrobial and pharmacokinetic characteristics, are under investigation, and important trials have been reported in the last 2 yr. These studies seem to suggest the relevant role of these new drugs in the antifungal armamentarium. The triazole voriconazole and the echinocandin caspofungin demonstrated significant advantages in term of toxicity and response rate, when compared with conventional amphotericin B in the treatment of aspergillosis and candidiasis, resp.Several substances with their cas registry numbers 162808-62-0 and 137234-62-9 may be metioned in this study. However, because of the peculiar characteristics of the design of both studies, the interpretation of the results may be difficult, and further confirmatory experiences are needed. Other noncontrolled studies showed a promising role of caspofungin in the treatment of Aspergillus infections and of voriconazole in the treatment of other rare mycoses. The increasing incidence and severity of invasive mycoses have led to the development of new strategies with new antifungal agents, and amphotericin B no longer is the gold std. for a variety of fungal infections. However, these results should be considered with caution. The latest generation drugs need to be investigated further in proper trials, and old drugs, esp. conventional amphotericin B, continue to have a central role in the antifungal armamentarium. .

A high-performance liq. chromatog. method for the quant. detn. of caspofungin in human microdialyzates with amperometric detection is described. Since microdialysis of caspofungin is performed with a 100,000 mol. mass cut-off membrane, microdialyzates contain protein that was pptd. at pH 4 with acetonitrile. Addn. of 1-propanol (33%, vol./vol.) to the sample ext. improved the anal. recovery to 81-89%. Caspofungin and the internal std. clarithromycin were sepd. isocratically on a cyanopropyl silica column using acetonitrile-0.05 M citrate (, vol./vol.), adjusted to an apparent pH of 6.9, at a flow rate of 1.0 mL/min, and amperometric detection at +950 mV oxidn. potential. Within-day and between-day imprecision and inaccuracy were <11%. The lower limit of quantification was 0.07 mg/mL. The method was applied to in vitro microdialysis expts. Ringer's soln. contg. 1% (w/v) human albumin was used for the perfusing and surrounding medium, resp. Albumin did not entirely prevent adsorption of caspofungin to the surface of membrane and/or tubing. When the binding-sites were satd. with albumin plus caspofungin prior to the start of sampling, the percentage of drug appearing in the microdialyzate ("recovery") remained stable over the concn. range tested.Except for chemicals metioned above, 162808-62-0 is also used. .
