DOCUMENT TYPE: Journal CA Section: 1 (Pharmacology) Na+ movements in human red blood cells were studied in order to test their sensitivity to inhibition by furosemide (I) [54-31-9] or bumetanide [28395-03-1]. Unidirectional Na+ influx and efflux values confirmed previous evidence for components of both these fluxes sensitive to inhibition by furosemide or bumetanide. The difference between furosemide-sensitive unidirectional influx and efflux was detd. at [Na]o 140 mM and shown to be nonsignificant, even when [Na]1 was raised to about 45 mmol/mL cells. No evidence was found for an influence of furosemide on net movements of Na+ in cells of either slightly elevated or physiol. [Na]u incubated in a 140 mM-Na+ medium over 4 h. Incubation of cells, under similar conditions, for 12 h did not reveal a bumetanide-sensitive net Na+ movement. The results suggest that cells with normal or slightly elevated [Na]1, incubated in media of physiol. Na+ concn., do not possess a mechanism sensitive to inhibition by the loop diuretics that operates to regulate cell Na+.

This apparent time lag reflects the disequil. between the urine concn. and effect compartment (biophase) which occurs during the early distribution phase. An expanded pharmacodynamic model has been introduced in which the hypothetical effect compartment is linked, by a first-order process (Kue), to the urine compartment. Drug dissipation from the effect compartment occurs by means of the first-order rate const, Keo. This representation accommodates bumetamide's luminal site of action in the kidney tubule as well as the drug's temporal component. Application of this model to the bumetanide-indomethacin [53-86-1] interaction in dogs is examd.
