DOCUMENT TYPE: Journal CA Section: 1 (Pharmacology) Increasing the urinary output of free thiol groups protects against cyclophosphamide [50-18-0]-induced bladder toxicity. In the present study, intact rats, isolated perfused kidneys, and freshly isolated cells from various rat organs are used to compare the efficacy of N-acetylcysteine [616-91-1] and 2-mercaptoethanesulfonic acid (mesna) [3375-50-6] as sources of urinary thiols. In intact rats given a single i.v. dose of mesna, urinary thiol output is approx. 10-fold higher than in rats given an equimolar dose of N-acetylcysteine. This is partly due to the fact that N-acetylcysteine is rapidly absorbed by various types of cells, whereas mesna is transported selectively to the kidney, and partly to different renal handling of the two compds. The results suggest that mesna is a more favorable drug than N-acetylcysteine for increasing urinary thiol excretion.

DOCUMENT TYPE: Journal CA Section: 1 (Pharmacology) 2-Mercaptoethanesulfonate (MESNA) [3375-50-6], a uroprotective agent, generally given to prevent hemorrhagic cystitis during oxazaphosphorine cancer chemotherapy, administered orally to volunteers was absorbed from the gastrointestinal tract and excreted in the urine (about 41.5% of the dose); peak excretion occurred 2-3 h after administration. A proportion of the excreted dose is as free thiols (about 24.2%) and the remainder is as disulfides. MESNA is shown to enhance excretion of cysteine [52-90-4] in urine.
