Hydrocortisone and betamethasone 17-valerate were absorbed into the skin via transfollicular and transepidermal routes, while the remaining 4 corticosteroids were absorbed mostly via a transfollicular route in normal human skin. The systemic effects of 4 corticosteroid prepns.; 17.alpha.-desoxymethasone [382-67-2] 17-valerate and flumethasone pivalate [2002-29-1], applied to normal and pathol. skin were investigated. Whole body application of the corticosteroids with or without occlusive dressing to diseased skin induced systemic effects assocd. with pituitary-adrenal suppression.

DOCUMENT TYPE: Journal CA Section: 2 (Mammalian Hormones) The effects of occlusive dressing with topical corticosteroids on the count, size, and morphol. of corneocytes in normal human skin were studied in 16 male volunteers, aged 23-25 yr, without skin lesions. They were divided into 2 groups according to duration of occlusive dressing: 3 days in group I and 6 days in group II. Specimens were obtained from 4 sites on the back, which were studied for the effect of occlusive dressings without any topicals, with 0.1% hydrocortisone-17-butyrate [13609-67-1] cream, with 0.25% desoxymethasone [382-67-2] ointment, and with ointment base, before and every 3-5 days after occlusive dressings, up to a total 4 times in group I and 5 times in group II. The mean no. and size of corneocytes in the control group were 124,682/cm2 and 1109. m2, resp. In the morphol. classification, the ratio of pentagonal to hexagonal to irregular cells was 29 to 19 to 52% in the control group. In group I, the no. of corneocytes decreased to 16.6% of control immediately after cessation of occlusive dressing without any topicals, and then gradually increased to 111.9% of the control on the 15th day and 111.7% on the 18th day. But in occlusive dressings with hydrocortisone, desoxymethasone, or base only, the no. of corneocytes decreased to 11.5%, 6.4%, and 10.1% of control immediately after cessation of occlusive dressing, and then increased gradually until the 18th day but did not reach the level of the control. In group II, the no. of corneocytes decreased, more remarkably than in group I, to <5% of the control immediately after cessation of occlusive dressing. Subsequently the no. of corneocytes increased above the normal level of the control of the 15th day and then decreased on the 27th day to 42.2% with no topicals, 19.5% with hydrocortisone, 16.1% with desoxymethasone, and 30.1% with base only. There were no statistically significant differences in cell size and morphol. throughout the expt.
