DOCUMENT TYPE: Journal CA Section: 1 (Pharmacology) The effect of several recently synthesized cardiotonic agents amrinone [60719-84-8], WIN 47203 [78415-72-2], AR-L 115 (vardax) [73384-60-8], AR-L 57 [53929-89-8], carbazeran [70724-25-3], and RMI 82249 [77671-29-5] on a variety of key metabolic processes involved in regulating Ca homeostasis in the cardiac cell was evaluated in lab. animals. Amrinone and vardax, at concns. that stimulate myocardial contractility, had no effect on Ca transport, uptake or EGTA-induced Ca release from isolated sarcoplasmic reticulum vesicles, or on mitochondrial Ca transport or uptake. Amrinone, vardax, and RMI 82249 also had no effect on mitochondrial respiration. AR-L 57 and vardax, but not amrinone, WIN 47203, or carbazeran, inhibited cardiac Na+, K+-ATPase [9000-83-3] activity (IC50 = 140 mM and approx. 1000 mM, resp.). All of the agents inhibited phosphodiesterase [9025-82-5] activity; WIN 47,203 and carbazeran were the most potent inhibitors, and amrinone, the least. Vardax, amrinone, and carbazeran also potentiated the inotropic response to the b-receptor stimulant isoproterenol, and these 3 agents, in addn. to WIN 47203, also elevated cyclic AMP [60-92-4] levels in isolated rabbit papillary muscles. Inhibition of phosphodiesterase activity may represent the cardiotonic mechanism of action of amrinone, WIN 47203, and carbazeran, and perhaps AR-L 57 and vardax as well.

Amrinone blunted the deleterious effects of enflurane or isoflurane on myocardial function, reduced systemic vascular resistance (SVR) and pulmonary capillary wedge pressure, restored left ventricular dP/dtmax, and maintained cardiac index, even at the high concns. of enflurane or isoflurane that caused marked cardiac depression. However, with reduced SVR, main arterial pressure remained low. No arrhythmias were obsd. at any time in this study. Amrinone did not increase plasma catecholamine levels with either anesthetic. Elevated amrinone plasma concns. persisted for at least 1 h after cessation of the high amrinone infusion and continued stimulatory effects on the cardiovascular system were obsd. These results suggest that in selected patients amrinone may be beneficial in the short-term treatment of perioperative depression of cardiac performance. .
