Ca di-Na EDTA [62-33-9] is often used therapeutically to reduce the body burden of Pb. Although this chelating agent promotes increased urinary excretion of Pb and lowers blood Pb concn., it is a nonspecific metal chelator and it promotes a substantial increase in the urinary loss of the essential metal, Zn. The pattern and extent of changes in blood Pb and plasma Zn and Cu concns. and of the urinary loss of these metals were characterized in Pb-poisoned children receiving 5-day courses of 1000 mg Ca di-Na EDTA/m2 of surface area/day, given i.m. Both blood Pb and plasma Zn concns. 7440-50-8 and 7440-66-6 are cas registry numbers of chemicals which are used as reagents here. were reduced rapidly during treatment; plasma Cu concn. was unaffected. At 60 h after the cessation of Ca di-Na EDTA treatment, plasma Zn concn. rebounded rapidly to its pretreatment value but blood Pb concn. remained at ~60% of its pretreatment value. Treatment with Ca di-Na EDTA produced an ~21-fold increase in the daily rate of urinary loss of Pb and an ~17-fold increase in the daily rate of Zn loss in urine. There was no increase in urinary loss of Cu above the endogenous level. The chelatable body burden of Pb was estd. by 2 related methods. Significant correlations were found between chelatable Pb burdens estd. by either method and blood Pb concns. before Ca di-Na EDTA treatment. In contrast, no correlation was found between plasma Zn concn. and the urinary loss of Zn during chelation therapy. Taken together, these data emphasize the nonspecificity of metal decorporation during Ca di-Na EDTA therapy and suggest the careful monitoring of Zn status is needed during treatment with this chelating agent. .
