Novel cystathionine metabolites, cystathionine sulfoxide and NAc-cystathionine sulfoxide, were identified in the urine of a patient with cystathioninuria using liquid chromatography/mass spectrometry with an atmospheric pressure chemical ionization interface system. NAc-cystathionine sulfoxide was discovered as a new metabolite of cystathionine in the urine of a cystathioninuric patient. The concentrations of cystathionine sulfoxide and NAc-cystathionine sulfoxide in this urine were also determined for the first time using this method, and were 28.69 ± 0.44 and 10.51 ± 0.49 mg g-1 creatinine, respectively. Neither compound was detected in the urine of healthy subjects.
