The research focuses on the development and evaluation of a new radiotracer, 4-[18F]Fluoro-m-hydroxyphenethylguanidine ([18F]4FMHPG, [18F]1), for quantifying regional cardiac sympathetic nerve density using positron emission tomography (PET). The study involved the automated synthesis of [18F]1, where the intermediate 4-[18F]fluoro-m-tyramine ([18F]16) was prepared using a diaryliodonium salt precursor for nucleophilic aromatic [18F]fluorination. The radiotracer was tested in isolated rat heart studies and microPET imaging studies in rhesus macaque monkeys, demonstrating high-quality cardiac images and specific uptake in cardiac sympathetic neurons. The kinetics of [18F]1 were analyzed using compartmental modeling and Patlak graphical analysis, providing quantitative measures of nerve density. The study also explored the potential of [18F]1 for oncology applications, particularly in localizing neuroendocrine tumors, and provided human radiation dosimetry estimates based on biodistribution studies in rats.
