Prekallikrein, also known as plasma prekallikrein or Fletcher factor, is a serine protease zymogen that plays a crucial role in the intrinsic pathway of blood coagulation and the kinin-generating system. It is synthesized in the liver and circulates in the blood as an inactive precursor, which is then converted to its active form, kallikrein, through the action of factor XIIa. Once activated, kallikrein contributes to the formation of bradykinin, a potent vasodilator, and also cleaves high molecular weight kininogen to release kinins. In the coagulation cascade, prekallikrein is involved in the activation of factor XI, which in turn leads to the activation of factor IX and the subsequent formation of thrombin, ultimately resulting in blood clot formation. Dysregulation of prekallikrein can lead to various bleeding disorders and is associated with certain hereditary conditions, such as Fletcher trait.
