neomycin is an aminoglycoside antibiotic found in many topical medications such as creams, ointments, and eyedrops. the discovery of neomycin dates back to 1949. it was discovered in the lab of selman waksman. neomycin belongs to aminoglycoside class of antibiotics that contain two or more aminosugars connected by glycosidic bonds.

applciation:

neomycin is typically used as a topical preparation, such as neosporin. it can also be given orally, where it is usually combined with other antibiotics. neomycin is not absorbed from the gastrointestinal tract and has been used as a preventive measure for hepatic encephalopathy andhypercholesterolemia. by killing bacteria in the intestinal tract, it keeps ammonia levels low and prevents hepatic encephalopathy, especially prior to gi surgery. it has also been used to treat small intestinal bacterial overgrowth. it is not given via injection, as neomycin is extremely nephrotoxic (causes kidney damage), even when compared to otheraminoglycosides. the exception is when neomycin is included, in very small quantities, as a preservative in some vaccines - typically 0.025 mg per dose.

indications and usage for neomycin sulfate:

to reduce the development of drug-resistant bacteria and maintain the effectiveness of neomycin sulfate tablets and other antibacterial drugs, neomycin sulfate tablets should be used only to treat or prevent infections that are proven or strongly suspected to be caused by susceptible bacteria. when culture and susceptibility information are available, they should be considered in selecting or modifying antibacterial therapy. in the absence of such data, local epidemiology and susceptibility patterns may contribute to the empiric selection of therapy.


suppression of intestinal bacteria:

neomycin sulfate tablets are indicated as adjunctive therapy as part of a regimen for the suppression of the normal bacterial flora of the bowel, e.g., preoperative preparation of the bowel. it is given concomitantly with erythromycin enteric-coated base

hepatic coma (portal-systemic encephalopathy)
neomycin sulfate has been shown to be effective adjunctive therapy in hepatic coma by reduction of the ammonia-forming bacteria in the intestinal tract. the subsequent reduction in blood ammonia has resulted in neurologic improvement.


contraindications:

neomycin sulfate oral preparations are contraindicated in the presence of intestinal obstruction and in individuals with a history of hypersensitivity to the drug.

patients with a history of hypersensitivity or serious toxic reaction to other aminoglycosides may have a cross-sensitivity to neomycin. neomycin sulfate oral preparations are contraindicated in patients with inflammatory or ulcerative gastrointestinal disease because of the potential for enhanced gastrointestinal absorption of neomycin.
