  • Chemical Structure and Mechanism of Action Tranexamic acid (TXA) is a synthetic derivative of the amino acid lysine. It competitively inhibits the activation of plasminogen to the serine protease, plasmin, via binding to kringle domains. TXA is also a competitive inhibitor of tissue plasminogen activator and directly inhibits plasmin activity.
  • Medical Applications Reduces the risk of bleeding among patients undergoing cardiac surgery.
    Effective in reducing perioperative bleeding in patients undergoing noncardiac surgery.
    Used in surgery to decrease bleeding and the need for blood transfusion.
    Available in intravenous and oral formulations.
  • Pharmacokinetics Distributed throughout all body tissues with a plasma half-life of 120 minutes.
    Bioavailability of oral and intravenous TXA is reported to be 33%-34%.
    Elimination is primarily through renal clearance.
  • FDA Approval and Off-Label Uses FDA-approved usage includes heavy menstrual bleeding and short-term prevention in patients with hemophilia.
    Off-label uses include management of bleeding trauma patients, postpartum hemorrhage, and surgical operations to reduce blood loss.
    TXA administration reduces mortality in bleeding trauma patients and postpartum hemorrhage.
  • Historical Background First made in 1962 by Japanese researchers Shosuke and Utako Okamoto. Listed on the World Health Organization's List of Essential Medicines. Approved by the US FDA in 1986.
