use:
it is mainly used as an antidote to folic acid antagonists such as methotrexate, pyrimethamine or trimethoprim.
used to prevent severe toxic effects caused by excessive or large doses of methotrexate.
megaloblastic anemia caused by folate deficiency.
when combined with fluorouracil, it is used to treat advanced colon cancer and rectal cancer.
precautions:
1. in the "rescue" treatment of methotrexate, if acidic urine (ph less than 7), ascites, dehydration, gastrointestinal obstruction, pleural effusion or renal dysfunction and other performance should be used with caution.
2.before and after administration and when using drugs, check or monitor: (1)before treatment, creatinine clearance test should be done; (2) the concentration of methotrexate in plasma or serum should be measured every 12~24h after large dose of calcium folinate. adjust the amount of calcium folinate; when the plasma or serum concentration is lower than 5×10 mol/l, laboratory monitoring can be stopped; (3) the serum creatinine should be measured every 24 hours before and after treatment. if the serum creatinine level is more than 24 hours after treatment increased by 50% before, suggesting severe nephrotoxicity, should be treated with caution; (4) before and after administration of leucovorin should monitor the urine acidity every 6 hours, require urine ph greater than 7, should be used routinely with sodium bicarbonate and hydration treatment (infusion volume per day is 3000ml/m).
3. if the daily amount of calcium folinate is more than 25mg, intramuscular injection should be used because the absorbed saturated dose for oral administration is 25mg per day.
4. each time a large dose of methotrexate is used for 24-48 hours and then leucovorin is used, the dose should be such that the blood drug concentration is equal to or greater than the blood drug concentration when methotrexate is used.
