model should facilitate treatment of patients after TIPS
placement because their prognoses can be predicted more
accurately. The present study identified a subgroup of
high-risk cirrhotic patients with ϳ90% mortality within
a few weeks after TIPS placement. Accordingly, the
appropriateness of TIPS placement in such high-risk
patients should be questioned (especially if they cannot
receive liver transplants). In addition, stratification of
patients in controlled trials before they are randomized
may help better define which patients are most likely to
benefit from TIPS compared with other forms of therapy.
