cantly suppressing PTH levels and bone alkaline
phosphatase and increasing serum calcium lev-
els. The variability in responses observed on this
study indicate that any potential immunomodula-
tory effect of vitamin D in dialysis patients is
masked by many other factors. That said, should
we be alarmed that many of our dialysis patients
are deficient in the essential vitamin (or hor-
mone) D? This may be answered by the VDR
knockout mouse in which secondary hyperpara-
thyroidism, bone rickets, and alopecia are the
only abnormalities noted and survival is unaf-
fected.40 Furthermore, in patients with many dif-
ferent types of rickets, skeletal growth is ad-
versely affected, but without obvious clinically
significant immune deficits.41 Thus, at the present
time, in patients not on vitamin D therapy for the
treatment of secondary hyperparathyroidism,
widespread use of vitamin D supplements for
potential immunomodulatory effects cannot be
recommended.
