DOCUMENT TYPE: Journal CA Section: 1 (Pharmacology) Glycopyrrolate methobromide (I) [596-51-0], which is frequently used to prevent central effects of anesthetics, inhibited human plasma cholinesterase (E.C. 3.1.1.8) (PChE) [9001-08-5] at an I50 (50% inhibitory concn.) of 1.0 mM; the I50 of atropine for PChE was 0.9 mM. Since I and atropine possess low inhibitory effects on human PChE activity, it is unlikely that PChE in vivo would be inhibited to an extent that interferes with the hydrolysis of succinylcholine and/or local anesthetics of the ester-type. Since I does not cross the blood-brain barrier, it should be void of central anticholinesterase effects such as inhibition of brain PChE at the receptor site. This lack of central effect of I make it a choice of anticholinergic drugs in anesthetic practice in patients in whom this interaction should be avoided.

The procedure involves preliminary ion-pair extn. of the drug into CH2Cl2, followed by concn. and anal. of the ion-pair complex by capillary column gas chromatog. using a N-sensitive detector. The method depends on the thermal dequaternization of the quaternary ammonium compd. and can be used to detect 5 ng/mL in a 3-mL plasma sample. The assay procedure has been applied to the detn. of the plasma concn. of glycopyrronium after i.v. administration to an anesthetized patient.
