The undecapeptide substance P (SP) belongs to the tachykinin family of regulatory peptides. SP derives from the TAC1 gene and binds to the neurokinin (NK)-1 receptor. The distribution of the SP/NK-1 receptor system and the functional significance of isoforms (full-length and truncated) of the NK-1 receptor will be mentioned. The physiological roles played by SP have been up-dated as well as the involvement of the undecapeptide in human diseases. SP plays an important role in many pathophysiological actions: pain, neurogenic inflammation, emesis, alcohol addiction, depression, viral/bacterial infection, cancer, asthma, arthritis, schizophrenia, smoking. Under pathological conditions, the SP/NK-1 receptor system is up-regulated. The potential therapeutic use of NK-1 receptor antagonists is also mentioned.
