Zileuton is a reversible inhibitor of 5-lipoxygenase, an iron containing enzyme which is synonymous with arachidonate 5-lipoxygenase. Zileuton is approved as a prophylactic agent for chronic treatment of asthma in adults and children 12 years of age or older. Zileuton is not used in an acute asthma attack to reverse bronchospasms. Preparations of zileuton include an immediate and extended release formulations. Major adverse effects associated with zileuton are behavioral and liver abnormalities. Liver toxicity is monitored using increased ALT (alanine transaminase) levels above baseline. Behavior changes that have been reported include agitation, aggression and hallucinations.
